Through Physical Consent Cards

At community events, broker offices, and employer wellness programs, we may collect your name, phone number, email address, and signature through a signed physical consent card. By signing, you authorize Hampi Health to contact you for healthcare navigation purposes.

Cookies and Website Usage Data

We may use cookies or similar technologies to analyze site usage and improve your experience. These collect non-personal data such as IP address, browser type, and pages visited. “I authorize Hampi Health to call or text me at the phone number and email provided (even if that number is on any Do Not Call Registry or is a wireless number) regarding healthcare navigation services, including assistance scheduling appointments with a primary care provider, clinic referrals, and health education resources in the Rio Grande Valley. In addition, I authorize Hampi Health to contact me for any relevant communication, follow-up, and health awareness purposes. I understand that I am not required to opt in to receive text messages, calls, or emails as a condition (directly or indirectly) of receiving services from Hampi Health. I am the current authorized user of the phone number and email address provided. I consent to calls being recorded and monitored. I understand that text messages are recurring and ongoing and that messaging and data rates may apply. I consent to and understand that emails are recurring and ongoing unless I choose to unsubscribe. I understand that I can withdraw consent or unsubscribe at any time.”


Information We Do Not Collect

Hampi Health does not collect or store Protected Health Information (PHI) beyond what is minimally necessary for navigation and scheduling coordination. We do not collect payment information, Social Security numbers, government ID numbers, or clinical health records through our website or navigation services.
