Hampi Health is a community health navigation platform dedicated to connecting Rio Grande Valley residents to trusted local primary care providers. We serve individuals, healthcare clinics, insurance brokers, and employers across Hidalgo, Cameron, Starr, and Willacy counties.

2.2 Through Patient Consent Forms (JotForm)
When Hampi Health engages in direct patient navigation services, we collect personal contact information — including name, phone number, and email address — through encrypted consent forms hosted on JotForm, a HIPAA-compliant third-party platform. This information is collected only after the individual has provided written authorization via our Patient Communication and Contact Authorization form.

We do not store or transmit Protected Health Information (PHI) beyond what is minimally necessary for navigation and scheduling coordination. 2.5 Patient Communication and Contact Authorization Language
When individuals provide consent to be contacted by Hampi Health — whether through our online JotForm consent form or a signed physical consent card — the following authorization language applies:
“I authorize Hampi Health to call or text me at the phone number and email provided (even if that number is on any Do Not Call Registry or is a wireless number) regarding healthcare navigation services, including assistance scheduling appointments with a primary care provider, clinic referrals, and health education resources in the Rio Grande Valley. In addition, I authorize Hampi Health to contact me for any relevant communication, follow-up, and health awareness purposes. I understand that I am not required to opt in to receive text messages, calls, or emails as a condition (directly or indirectly) of receiving services from Hampi Health. I am the current authorized user of the phone number and email address provided. I consent to calls being recorded and monitored. I understand that text messages are recurring and ongoing and that messaging and data rates may apply. I consent to and understand that emails are recurring and ongoing unless I choose to unsubscribe. I understand that I can withdraw consent or unsubscribe at any time.”
3. Community Health Navigation Services
Hampi Health operates as a community health navigation platform. Our navigation services are provided by a community health navigator and are educational and logistical in nature.

Hampi Health is not a licensed healthcare provider, insurance broker, or benefits counselor. Navigation services are provided as a free community service. Any medical decisions remain the sole responsibility of the individual and their healthcare provider.
4. Primary Care Directory
4.1 For Community Members
Our Primary Care Directory connects RGV residents with verified local healthcare providers. Directory listings include contact information, hours, insurance accepted, and services offered. While Hampi Health verifies listing accuracy at the time of onboarding, we recommend confirming details directly with the provider before scheduling. Hampi Health is not responsible for the quality, availability, or outcome of provider services.
4.2 For Healthcare Providers
Providers may apply to list their clinic through the Hampi Health Provider Application Form. Listings are subject to our verification process. We reserve the right to approve, reject, or remove listings at our discretion. Subscription-based listing tiers are available. Clinic partners agree to maintain accurate information and notify Hampi Health within 5 business days of any changes to clinic details.
5. Clinic Partnership Program
Healthcare providers may enter into a formal clinic partnership with Hampi Health through a written service agreement. Clinic partners receive enhanced directory visibility, priority inclusion in navigator community outreach, and access to Hampi Health’s employer wellness referral network.
Clinic partnership fees are subscription-based and billed monthly. Partnerships may be terminated by either party with 30 days written notice. Hampi Health does not guarantee a specific number of patient connections as a condition of partnership. Compensation is charged for marketing visibility and navigator outreach services — not for patient referrals.
6. Broker and Employer Partnership Programs
6.1 Insurance Broker Partnerships
Hampi Health partners with licensed insurance brokers to support their members in accessing primary care following enrollment. Broker partnerships are governed by a separate written service agreement. Hampi Health provides community education and navigation services to broker-referred members — it does not act as an insurance representative, receive compensation per patient referred, or engage in activities that would require an insurance license.
6.2 Employer Wellness Programs
Hampi Health partners with employers to deliver the Health Savings Program — a structured employee wellness initiative that includes quarterly health events, navigation services, a doc series, and monthly impact reporting. Employer wellness programs are governed by a separate written agreement. Hampi Health does not share individual employee health information with employers. All reports are aggregate and anonymized.
7. Use of Our Website and Services

10. Disclaimer of Warranties
Our website and services are provided “as is” without warranties of any kind, express or implied, including accuracy, reliability, or fitness for a particular purpose. We do not guarantee that our website will be uninterrupted or error-free. Provider listings are verified at the time of onboarding but may change. Use our services at your own risk, as permitted by Texas law.

To the fullest extent permitted by Texas law, Hampi Health, its affiliates, and partners are not liable for any direct, indirect, incidental, or consequential damages arising from your use of our website or services, including losses from provider interactions, reliance on directory information, or outcomes of navigation services. Hampi Health is a navigation and education service — all medical, insurance, and clinical decisions remain the responsibility of the individual and their providers.

You agree to indemnify and hold Hampi Health harmless from any claims, damages, or losses arising from your violation of these terms, misuse of our website or services, or interactions with third parties connected through our platform.
13. Changes to These Terms
We may update these Terms and Conditions to reflect changes in our services or legal requirements. We will notify users via our website at least 30 days before significant changes take effect. Continued use of our website or services after changes constitutes acceptance of the updated terms.
